Required Health Certificate Information

To process a health certificate, your horse must be evaluated by one of our veterinarians and we must have the following information.  To ensure the timely completion of your horse's certificate, please submit the following information as early as possible and allow two business days from the date of the evaluation for processing.    Health certificates needed within two business days will incur a $30.00 rush fee.
