Veterinary student externs must register with NC Veterinary Medical board.  The board must receive this paperwork AT LEAST 15 days before the externship begins, so be sure to get an early start.

Log onto the NC Veterinary Medical Board website: http://www.ncvmb.org

Choose the "Applications and Forms" button on the left.

Choose "Application for Interns/Preceptees".

Fill out the form in its entirety.  Please note that the form requires the signature of the supervising veterinarian.  You may fax it to Carolina Equine Hospital to be signed and we will fax it back to you, OR you may send the form with check, photo and pre-addressed envelope to Carolina Equine Hospital to be signed and we will forward it directly to the board.

All students should email: info@carolinaequinehospital.com approx 1-2 weeks prior to the beginning of their externship at CEH. We will issue you a password so that you may download important student documents on our site.
